Tuesday: 8:30 AM to 11:15 AM; 1:30 PM to 4 PM
Thursday: 8:30 AM to 11:15 AM; 1:30 PM to 4 PM
Can only be scheduled after your initial consult. Procedure days are Mondays and Wednesdays.
Once you have made your initial appointment, you can pre-register by clicking on the links below to complete and submit the New Patient Forms or you may download, print, and complete the forms prior to your arrival. This will help to speed up the new patient registration process and get you in to see the doctor faster!
